JOB DESCRIPTIONThis Telephonic Case Manager RN Position Features:
? New opportunity for experienced Case Manager RN's!
Telephonic/work from home position, 40 hours a week Mon-Fri 8a-5p.
Hiring RN's with a minimum of three years clinical experience at the facility level as well as experience in RN Case Management, Home Health, Discharge Planning, Catastrophic Case Management, SNF/LTAC , Utilization Review, Transition Care and Managed Care.
Must have experience with the following job responsibilities
- Plan, implement, coordinate and monitor Case Management activities that may include acute and non-acute services, outpatient services and or community resources for high risk members.
? Collaborate with members interdisciplinary teams including Member?s Physicians, Plan Medical Directors, Managers, Local, Regional and Specialty team members
? Health and medication education, encouraging a healthy lifestyle and behavior changes for chronic patients
? Knowledge of Medicare/Medicaid in order to educate regarding benefits, identify candidates for Care Programs
Clear/Active FL RN License
Minimum 3 years of experience in acute care
Minimum 3 years of experience in Case Management or Home Health as an RN
Managed Care knowledge/experience
Computer savvy Must have high speed, hard wired internet and home computer
BSN highly preferred
Bilingual Spanish highly preferred
Applicants with previous experience working as a remote telephonic Case Manager RN with a Managed Care company highly desired/preferred.
