JOB DESCRIPTIONJob Summary
The Post-transplant Coordinator I facilitates and coordinates all aspects of the peri-operative and post-transplant phases of transplantation. The coordinator interacts with all members of multi-disciplinary transplant team to ensure coordination of care for the post-transplant patient.
Duties and Responsibilities are cross-referenced to the hospital s Pillars of Excellence and will be transferred to the Performance Evaluation.
* Communicates in an active, positive, and effective manner to all health care team members and reports pertinent patient care and family data in a comprehensive and unbiased manner, listens and responds to the ideas of others.
* Communicates with patient, referring physicians, other health care providers, payors, and transplant team members regarding the patient s status and maintains communication as needed.
* Uses therapeutic communication to establish a relationship with patients and families and communicates the plan of care. Identifies and responds to the clinical, psychosocial and economic needs of the transplant patient/family/caregiver during pre-operative, hospital, and post-transplant phases.
* Provides staff and community education in the areas of transplant and organ donation. Participates in department donor awareness events, educational opportunities, and community activities.
* Presents a professional image at work and all work-related events. Fosters teamwork approach in all interactions with peers and team members. Anticipates needs of other team members; proactively offers assistance.
* Contributes to meeting departmental and organ program targets for patient satisfaction measures on departmental dashboard.
* Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure proper department financial management.
* Contributes to meeting departmental and organ program financial measures on departmental dashboard.
1.Monitors patient progress post-transplant, identifies complications and considers interventions with the transplant team.
* Collaborates with nursing staff, social workers, and other health care providers to establish and implement patient care plan according to current standards of care.
3.Observes, monitors, assesses, and reports to appropriate health care provider the recipient s:
a.Physical and emotional status
b.Short and long-term complications of immunosuppression
e.Compliance with medications and medical regimen
* Demonstrates knowledge of, and compliance with, CMS Conditions of Participation and OPTN/UNOS policies. Provides data to OPTN/UNOS for recipient registration and follow-up (recipient registration form, recipient follow-up form, and malignancy form if indicated).
* Provides and maintains comprehensive documentation of donor/recipient transplant information and recipient s progress post transplantation.
* Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure compliance with all governmental reporting requirements, and proper quality outcomes monitoring.
* Contributes to meeting departmental and organ program targets for quality and safety, on departmental dashboard.
1.Establishes mutual educational goals post-transplantation with patient and family/caregivers and provides education and appropriate resources regarding
a.Vital signs, diet, and activity
b.Frequency of follow-up visits and lab studies
c.Reporting abnormalities to transplant center/primary care provider
d.Signs and symptoms of rejections/infection
e.Medications (purpose, dosage, frequency, and side effects)
f.Explanation of procedures required for follow-up
g.Need for periodic follow-up at transplant center
h.Importance of recipient/family/caregivers commitment to transplant medical and medication regimen
i.Importance of well-care (i.e. immunizations, mammography, pap smears, and colonoscopy)
j.Voluntary option of, and the process for, written communication to the donor family
2.Coordinates/ensures discharge planning to provide for recipient s continuity of care. Collects and reviews data pertinent to recipients health and organ function per protocol and as needed in follow-up. Communicates lab data and diagnostic results to transplant physician/health care provider. Implements changes in drug/treatment therapy as ordered by transplant physicians. Arranges for consultations, diagnostic procedures, and hospitalization when indicated. Maintains communication with referring physician.
* Participates in transplant event process according to transplant center protocol.
* Contributes to meeting departmental and organ program targets for process on departmental dashboard.

Applies evidence-based practice to daily work, such as reviewing the professional literature and attending professional meetings and sharing with colleagues.

Registered Nurse, Bachelor s degree required
Minimum 4 years nursing experience required. ICU experience preferred. Previous transplant acute care experience is an asset. Prior Transplant Coordinator experience not required.
CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
Registered nurse with current Texas license
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
Skilled at managing complex patient caseload and ability to prioritize/triage based on patient status.
Extremely well-organized and detail oriented, self-directed.
Excellent interpersonal skills required to deal with diverse patient population.
Excellent oral and written communication skills required.
