Full time position available as a Medical Assistant Coordinator. Hours are generally Monday through Friday 12-8 and may require weekend coverage as well.

General Summary/Overview:

The Medical Assistant Flow Coordinator works as part of the patient-centered healthcare team to manage patient population, anticipate patient needs, assist to identify and ensure relevant tests and patient information is timely available for physicians, while providing real-time care and customer service to our patients in the office, telephonically, and through Patient Gateway.

Principal Duties and Responsibilities:

Works closely with the physician/provider to appropriately schedule patients to maximize efficiency and timely provide patient care and patient services.
Responsible for patient and work flow management on a day-to-day basis to prioritize and ensure that the provider and other healthcare team members handle messages, prescription refills, review of labs/results, and forms are completed timely to eliminate batching of work.
With oversight and direction from the physician/provider, coordinates pre-visit preparations for patient visits to include:
o Preparing summary reports and assisting in the evaluation of patient needs, ensuring relevant labs and tests are scheduled and completed prior to office visits as required and anticipate special needs for effective communication to service vulnerable populations,
o Ensuring all required documentation is in the electronic medical record prior to the office visit,
o Identifying high risk, managed care, or patients on population management registry to ensure appropriate protocols are followed,
o Communicate with healthcare team as appropriate to ensure patient needs are met prior to the visit. Participates in the healthcare team huddle.
o Cleaning, preparing, and ensuring supplies are appropriately stocked and ready for use in the exam room and clinical areas. Cleans and sanitizes as needed.
At time of visit:
o Rooms patient and takes vitals.
o Assists with patient data collection using established guidelines and tools (pre-visit form, reason for visit, medical reconciliation, health monitoring, other providers, other updates) to flag changes and new information for provider. Agenda sets visit with patient.
o Utilizes effective communication skills with patient to address the patients special needs.
Post-visit, works with the provider and healthcare team to ensure appropriate referrals/tests are booked real-time as possible and required prior authorizations are completed. Manages appropriate tracking for follow up for referrals/tests.
Runs reports and leads pro-active measures to minimize overdue appointments, tests, and specialists visits.
May follow written standing orders from physicians and nurse practitioners to refill medications if permitted by office guidelines and/or use RX Queue to initiate refill requests.
Communicates with care management staff to follow up on patients recently discharged from acute and subacute facilities.
Utilizes established clinical standards of excellence, age/population-appropriate communication skills, approved guidelines and clinical knowledge.
Provides front desk and phone support as needed including performing patient scheduling, check in/check out, referrals, prior-authorizations, and other administrative tasks.
Follows patient identification protocol, maintains confidentiality, and upholds compliance standards according to policy.
Ensures all orders and forms are complete.
May perform other tests and patient services including EKGs, phlebotomy, injections, PFTs, urine collection, or assist providers with procedures relevant to their experience/training. Follows protocols and procedures for testing and procedures as appropriate.
Other duties as assigned from time to time.

Knowledge of physician policies and procedures including general knowledge of billing, insurance coding, and medical terminology.
Strong computer and typing skills with previous MS Outlook and electronic scheduling/electronic medical record system preferred.
Ability to work safely in a patient care environment and follow OSHA and safety requirements.
Effective phlebotomy skills
