As a Waiver Manager, you will:* Understand the criteria for acceptance of patients, appropriate utilization and reimbursement mechanisms as they relate to the delivery of services to patients. Collaborate with the HCB State Coordinator regarding acceptance, assist in determining qualification and appropriateness for waiver services. * Schedule initial assessments, ensure that coordinators are notified of patient needs and start of care. Ensure that documentation of initial assessments is complete and meets established timeframes. * Monitor compliance with care plans. Investigate all variances and takes appropriate action. * Monitor all high risk clients and keeps the Director informed regarding all potential negative outcomes. * Implement and approve changes to the patient's plan of care when needed in consultation with the Case Managers. Approve all verbal orders, notified coordinators of needed schedule changes, adds supplies as needed. * Ensure that re-certifications are completed in a timely manner and assures compliance with waiver regulations and timeframes. * Review all OASIS assessments for accuracy, completeness, and compliance with regulatory and organizational requirements. Assure that plans of care are congruent with OASIS assessments and are appropriate to meet client needs. * Investigate all customer service issues and brings appropriate resolution. Notify the Directors of Waiver and VS about all complaints and incidents and notified the corporate office as designated by policy. * Supervise the care managers and assist with problem solving difficult issues including referrals for CT skilled services and referrals to other agencies and community resources. * Monitor staff compliance with DVR and clinical note submission and approves time records for all direct reports. * Perform audits of nursing, care management, and HHA notes to assure compliance with company policy. * Participate in VS staff meetings. * Perform weekly review of payroll/billing to assure accuracy and cost effective staff utilization monitoring key indicators such as productivity, mileage, etc., and keep the Director informed when there are problems. * Coordinate and evaluate the delivery of services on a daily basis providing clinical oversight to direct care staff and supervision coordinators staffing and scheduling. * Provide on-site supervision for professional staff including annual observations and every three year credentialing. * Participate in recruitment and retention measures to secure and retain quality employees. * Conduct or assist with orientation of waiver employees. * Maintain knowledge and expertise in the area of home health nursing and demonstrate a commitment to professional growth and competence. * Maintain contact with referral sources to the waiver program. * Identify and develop new sources of referral for the program. * Demonstrate a commitment to the vision, mission, and goals of the organization by assuring the delivery of high quality services. * Supports the goal of continuous Quality Improvement by performing quarterly self-audits, handling complaints, making pertinent suggestions to improve efficiency, customer service, customer satisfaction, and/or cost containment. * Follow organizational policies and procedures regarding infection control, disposal of hazardous waste, and use of protective devices as evidenced by documentation and absence of related incident reports. * Support the principles of the Corporate Compliance Program, attends orientation and in-service updates and provides appropriate guidance to staff regarding program policies. * Perform other duties as assigned by the Director.
* An associate's degree in a health or human services field from an accredited college or university; AND At least one (1) year experience in a health or human services field; OR The educational or experiential equivalent in the field of aging or disabilities ; OR a licensed nurse from an accredited college or university; AND Minimum of one (1) year experience in a health or human services field.
* Possesses detailed knowledge of personal care rules and regulations including waiver services.
* Ability to read, write and speak English proficiently.
* Ability to work on computers proficiently and experience using Microsoft Office.
* Must be self-motivated and maintain a positive upbeat attitude.
* Possesses excellent verbal and written communicate skills with the ability to communicate across all levels of authority and to prospective clients
* Ability to successfully interact with people in a professional manner in face to face situations as well as by telephone.
* Previous experience in home care/Waiver program preferred.
* Knowledge of home care and Waiver regulations preferred.
