* Provides clinical pharmacy services for optimal, cost effective patient care specifically related to anticoagulation/critical care.
* Responsible for oversight and monitoring/data collection for patients receiving anticoagulants and adherence to hospital prescribing guidelines.
* Educates health professionals, pharmacy staff, patients and students through in-services and written/verbal materials.
* Good communication skills are essential for multidisciplinary interaction and education.
Scheduled Days / Hours: Weekdays;
Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy from accredited School of Pharmacy.
Experience: Hospital pharmacy experience (or clinically relevant experience) required.
PGY 1 required and PGY2 Critical Care Residency required; anticoagulation monitoring experience required.
