As a **Peritoneal Dialysis (PD) Home RN** , you educate your patients on how to do their own dialysis from their homes, and you take responsibility for the complete ongoing care of each patient to ensure their treatment stays on track. Through this experience, the trusting relationships you build with patients and their families are strong and fulfilling.

+ Assists in examination and evaluation of patient so that appropriate treatment and other services can be arranged.
+ Provides counseling and emotional support to minimize patient's anxiety over condition.
+ Keeps comprehensive records of the schedule and type of treatment and changes in the condition of the patient.
+ Checks dialysis equipment and ensures it is operating properly, making adjustments or having them made by maintenance personnel.
+ Monitors patient and equipment during dialysis and teaches patient to monitor own dialysis equipment.
+ Teaches patient proper care between dialysis to avoid infections, and instructs patient on matters of diet.
+ Assists in management of renal failure and responds to other emergency situations such as massive blood loss, shock, or equipment failure.
+ Make contact with the peritoneal dialysis patient at least monthly by phone or during clinic visits.
+ Obtain routine laboratory work to include routine adequacy testing.
+ Ensure the patient is provided consultation with a Social Worker and Dietitian.
+ Establish and maintain an ordering system that ensures the patient receives all supplies in a timely manner.
+ Minimum of 12 months' nephrology nursing experience preferred.
+ 3 months' experience in PD
+ Current driver's license, reliable transportation, and vehicle insurance in state of residence
+ Current CPR certification (or certification obtained within 60 days of hire)
+ Current RN license in state of practice
+ A.D.N degree from accredited school of nursing required
+ Experience in case management, home health, disease management, or hospice preferred
+ Industry-leading patient care with clinical outcomes ranked above the national average.
