The Director of Care Management, Post-Acute Care Services is responsible for strategic planning, development and implementation of UCSD s post-acute care (PAC) Preferred Provider Network and affiliated operations.

The Director establishes priorities, allocates resources and builds strategic partnerships to improve transitions of care across UCSD Health. The Director is responsible for managing network of providers (which includes home health, skilled nursing facilities, long-term care, and inpatient hospice providers) including contractual obligations, service level agreements. In addition, the Director leads activities to grow and/or modify the network as appropriate using a standardized performance management framework.

The Director works across hospitals and the ambulatory environment to achieve standardized, high-quality transitions to post-acute care throughout UCSD Health. Close collaboration with other initiatives and programs to improve care transitions is integral to the success of this position. The Director is a leader in UCSD s pursuit to transform healthcare to achieve the highest levels of quality, access, and affordability.

Provides oversight of post-acute care network of preferred providers and clinical liaisons onsite at UCSD. Conducts regular meeting with each preferred provider to review performance. Facilitates quality improvement calls/meeting between post-acute providers and UCSD providers. Manages quarterly Post-Acute Care Preferred Provider Collaborative meetings. Maintains oversight of network including changing or augmenting the network. Provides oversight of UCSD operations that occur within post-acute care facilities, including a SNFist program, which will provide clinical oversight and integration with partner SNFs.

Works closely with Care Management leaders to ensure that updated referral process is being adhered to. Facilitates resolution of any concerns Care Managers have with the preferred providers. Ensures collaboration on quality improvement with Care Management team and preferred providers. Provides insight into incorporating preferred providers into strategic initiative. Provides consistent oversight of key performance indicators (KPIs) for the preferred provider network. Manages variance from KPIs. Updates executive leadership on the performance on the network.



A Master s Degree in Business Administration, Hospital Administration, or related fields; or equivalent combination of education and experience.

At least ten (10) years of experience in hospital, provider, or health plan position with demonstrated experience in managing post-acute providers and/or external partnerships.

Demonstrated leadership in strategic and business planning, financial planning and analysis, program development and physician development required.

Experience in project management, development and facilitation of planning sessions with senior executives, directors/trustees, management, and consultants.

Knowledge regarding the healthcare market and government rules and regulations impacting post-acute care provider partnerships.

Understanding of business planning including analysis, statistics, budgeting, feasibility studies and implementation.

Exceptional oral and written communication skills; ability to communicate effectively with executives, nurses, physicians, and external partners.

Ability to gain cooperation and support through effective use of influence and persuasion.

Experience engaging and working in a team environment required.

Proven ability to work effectively with others as a team member and leadership skills to implement and execute system-wide care management operations.


Knowledge of the UC San Diego Health organization is desirable.
