Timely and accurate assessment of the patient/ client clinical, social, functional and continuing care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention. Documents functional, situational and clinical assessment within 24 hours of patient/client contact. Assist patients who have financial difficulties in obtaining medications needed or treatments needed for their medical care.
Be able to provide symptom Triage for patients of the department. Plans for and identifies immediate, short term, and long term care needs for patient/client. Sets appropriate patient/client goals and time frames for achievement in order to achieve desired outcomes. Establishes appropriate monitoring tools to measure outcomes related to client/patient interventions in order to continuously improve quality of services.
Monthly reporting of trends and outcomes that identifies patient/client populations. Monthly outcomes reporting of client/patient based data to client/patient based teams in order to improve quality of case management services. Capable of seen patients under Nurse visits (Blood pressure checks, UTIs and any previously ordered vaccines and therapeutic injections)
RN license issued by the state of California.
Bart or BLS (CPR) with commitment to get BART within 6 months of hire date.
Minimum of two (2) years of solid experience with Primary Care Internal Medicine.
Bilingual Familiarity with quality national standards and measures to improve patient care.
Case Management experience.
Capability of consistently making intelligent decisions under pressure.
Possess good judgment, initiative, have an excellent attitude and be dependable.
Excellent customer service.
Excellent writing skills.
Familiarity with quality national standards and measures to improve patient care.
Solid communication skills, strong critical and analytical thinking skills, and appreciation of diversity.
Must be able to work various hours and locations based on business needs.
Must be able to lift and push 20 lbs.
