Summary: The physical therapist contracted or employed through the Agency is responsible for providing physical therapy services and for adherence to all conditions outlined in policies and procedures. Provision, direction, supervision, and evaluation of physical therapy services to patients in the home in accordance with agency policies and procedures, applicable health care standards, and governmental laws and regulations.Requirements:
Licenses/Certifications:* Current licensure in state and CPR certification.* Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.* Active/current agency health requirements.* Must complete required department competencies to validate and maintain competency.Education/Experience:Education: Graduate of a physical therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association.Experience: At least one (1) year of appropriate experience as a physical therapist. Preferably in out-patient, acute care setting, Community/home health or skilled nursing facility experience is preferred.
