Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for performing skilled nursing functions in accordance with the State Nurse Practice Act and acceptable standards of practice, federal, state, and local laws and within the guidelines of his/her professional license. The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager . Graduate of an approved, accredited school of professional nursing required. Current licensure to practice as a Licensed Practical Nurse (LPN) in the Commonwealth of Massachusetts required. Current CPR certification required.

Preference will be given to applicants with one (1) year experience in a home care/hospice setting or equivalent experience required. Demonstrated knowledge of hom
health regulatory and reimbursement requirements required.

Demonstrated good written and verbal communication and interpersonal skills required. Demonstrated strong organizational and time management skills required.

Current drivers license, good driving record and reliable transportation required.
