Buffalo. BFNC, INC. Coordination/Mental Health Supported Apartment Program.
Manage day-to-day operations of licensed community based apartment program (24 beds). Lead and supervise staff of 2 Case Managers. Ensure apartments meet standards for habitability and safety; facilitate the development of customized ISP's, documentation of contact logs, meet Medicaid documentation standards, create consumer activity calendars and monitor participation, manage contracts for SAS, Symptom Management and clinical consults. Prepare weekly, monthly, quarterly statistical reports. Requires BA in social work or related field and one year of working with mental disorders AND one year of program management experience. Must have excellent communication and interpersonal skills, value collaboration, cultural difference, person-centered services. Must have a valid NYS driver's license. Full-time. Salary range is high twenties to low thirties.
