Clinical Social Worker - Care Coordination
This is a regular, hourly, variable (combo of 8 & 10) hour, Day shift position, per diem. This position is not eligible for benefits.
As a member of the Care Coordination team, the Clinical Social Worker is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, complex discharge planning, crisis intervention, referral and resource referral. Assists the patient and family transition to the next appropriate level of care.

Licensed Social Work Associate ıı" Advanced (LSWAA) or Licensed Social Work Associate ıı" Independent Clinical (LSWAIC) required. Licensure as an Independent Clinical Social Worker (LICSW) or Advanced Social Worker (LASW) with the Washington State Department of Health under Chapter 18.225 RCW is preferred and may be substituted.
Preference will be given to applicants with a minimum of one-year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting. Experience conducting psychosocial assessments with diverse client populations including various age groups who may experience physical or developmental disabilities, emotional, behavioral, and mental disorders, substance abuse. Experienced in conducting crisis interventions. Skills and knowledge base congruent with the Social Work Code of Ethics required. Demonstrated understanding and clinical knowledge of the grief process and family dynamics required. Skills in working with groups, ability to develop cooperative relationships with families, patients, physicians, and other health care professionals within and outside Swedish Medical Center required. Demonstrated ability to participate in social and community planning. Ability to use a computer for clinical data entry or retrieval. Demonstrated efficiency with a keyboard and mouse. Knowledgeable in the use of general office equipment (i.e. telephones, voice mail, fax machines and photocopiers). Excellent verbal communication skills in person and on the telephone. Ability to communicate effectively with staff of all job categories. Ability to read, write legibly, understand and communicate clearly in English. Demonstrated ability to use basic math skills. Ability to organize and prioritize work. Ability to act independently, utilizing critical thinking and problem solving skills. Must be a mature individual capable of independent decision-making and ability to work collaboratively with others in a variety of settings and disciplines. Demonstrated ability to work with a wide range of staff as part of an interdisciplinary team. Maintains skills and competencies defined in the unit specific orientation competency skill list.
