The NCM is responsible for directing an organized, patient-centered interdisciplinary team system that coordinates the delivery of healthcare services by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes and efficacious use of resources. The NCM coordinates with physicians, Case Management Social Worker, and other health team members to expedite medically appropriate, cost-effective care and promote quality through evidence based medical practice throughout the health care system. Licensure / Certification / Registration: Current licensure as a Registered Nurse in the State of Montana, Current CPR certification, current Case Manager Certification preferred. Experience: 3-5 years experience in clinical nursing. Hospital case management experience required. Special qualifications: Knowledge of current case management principles, third party payers requirements and discharge planning process. Knowledge of current nursing principles, management principles, as well as hospital organizational operations. Must be able to work calmly in stressful situations and be self-directed. Excellent organizational skills, as well as excellent and effective oral and written communications skills. Ability to adapt to constantly changing demands of outside payer groups. Demonstrates diplomacy, ability to collaborate with others and coordinate activities. Demonstrates good judgment and high degree of concentration. Ability to interpret information from medical records and make decisions. Knowledge of clinical databases and strong assessment skills.
