As a Rehabilitation Specialist for Rehab Without Walls®, you will:
*Implement treatment protocols established by registered/licensed clinician to assist the person served in achieving the individualized care/treatment plan goals. Collect clinical data on the person receiving services.
*Ensure safety, living comfort and daily hygiene of person receiving services while fostering independent functioning.
*Teach responsibility, independent functioning and pro-social behavior to model appropriate social and professional behavior. Encourage positive communication with others.
*Guide person served in daily medication routine as directed by licensed medical professional and as indicated by state law.
*Adhere to/enforce Bill of Rights of person receiving services and report any violation to supervisor.
*Implement community reintegration activities under the direction of a registered/licensed therapist to assist the person receiving services with a variety of needs during physical activity. Assist the person receiving services in understanding their needs and limitations and how to manage them during function.
*Observe/document and report changes in the condition of the person served to the Clinical Coordinator/designee, attending physician and other treatment team members as appropriate.
*Participate in the team process, provide input and effective treatment strategies. Attend case conference and family educational meetings.
*Attend, participate in and/or conduct internal staff development programs, obtain continuing education as required by Company policy and regulations. Maintain and enhance clinical practice skills.
Qualifications
*High School Diploma or the equivalent
*Plus a minimum of six months prior work/volunteer experience providing individual instructions, preferably in a health care environment is generally required
