The Physical Therapist orients patients, family and/or other appropriate parties to the purpose and procedures of the physical therapy evaluation and treatment; obtains and documents all required and pertinent patient information, including precautions and orders; selects evaluation and treatment methods which correlates to the age of the patient, the patient's diagnosis, symptoms and physician orders; completes an initial written assessment of referred patients within established time frames; documents an initial plan of care and completes certification form based on assessment results within one working day; updates outpatient plan of treatment and documents according to practice guidelines; facilitates family and patient involvement in the development of the treatment plan; documents the patient's response to treatment and progress; identifies significant changes in patient's condition and takes appropriate actions, as evidenced in reporting, staffing consultation and documentation; participates with inter-disciplinary team in the development and accurate documentation of a discharge plan; charges patient accurately according to treatment provided; meets requirements of third party payors to optimize reimbursements and meets productivity standards. In addition, the Physical Therapist reviews patient care plans and medical charts; coordinates schedules with disciplines; selects and provides treatment according to the patient's plan of care which correlates to the age of the patient, the practice guides and the physician's orders; provides appropriate patient education, family teaching and support which relates to the needs of the patient and family; reassesses patient's response to treatment and modifies treatment; directs cross-trainable activities and cares; monitors program effectiveness and outcomes, initiating programs and development of new programs. Patients cared for include those in the following age groups; infant, child, adolescent, adult and geriatric. KNOWLEDGE & SKILLS REQUIRED: Required: - Graduate of a Physical Therapy program. - Licensure required by the North Dakota Board of Physical Therapists. - Successful completion of the functional assessment.
