Position Summary: A social worker intervenes with patients who have complex psychosocial needs, which require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance from various agencies and special funds. In addition, offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for patients at risk. This role will receive referrals for individuals from interdisciplinary team members including physicians, case managers, staff nurses and other members involved in patient care. Qualifications: Masters in Social Work with LCSW licensure required. Two years of social work experience required.
