The Director of Case Management demonstrates twenty four hour responsibility and accountability in ensuring case management process is initiated effectively across the organization, and patients receive the appropriate level of care across the continuum.

The 307-bed facility is Joint Commission accredited and committed to providing high-quality medical care in a friendly hospital environment, offering a full range of services including emergency, surgical services, orthopedics, cancer, surgical weight management at the Florida Bariatrics Centers, wound care, a Rehabilitation Center and Sleep Disorders Center. The hospital also recently opened the area's first Acute Care for the Elderly (ACE) unit.

Completion of RN Diploma program or related health care program from accredited program, college or university
Four to five years previous Case Management experience; two years were in a management role.
Must possess current Florida State RN licensure.
