We are the primary teaching hospital for the Frank Netter School of Medicine at Quinnipiac University.Nurse Practitioner (APRN) Role ScopeSaint Vincent's Medical Center is seeking a nurse practitioner (APRN) to join a strong, cohesive and collaborative team of Palliative Care. The Palliative Care APRN will work in collaboration with a wide variety of resources to coordinate the most appropriate care and treatments for patients with end stage or chronic illness. He/she will obtain patients through referrals from the attending physician for the purpose of assessing a client's needs and wishes to maintain an optimal quality of life. He/she will educate the patient/family in order to allow them to make an informed decision to develop a plan of care, including the outcomes of the care to which they consent. He/she will advocate implementing a plan of care according to the wishes of the patient/family in coordination with the primary care physician.
