JOB DESCRIPTIONPOSITION DESCRIPTION
The Population Health Care Coordinator, under the direction of the Manager of Quality Improvement, has primary responsibility for care management activities for a specific patient panel. This will include identifying the high acuity patient population and developing, conducting, and monitoring care management processes. The Population Health Care Coordinator will work with the Manager of Quality Improvement to develop this position to best serve the needs of the patient population.POSITION REQUIREMENTS
Required: 1. Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting. 2. Proficient computer skills, including Microsoft Office (specifically Word and Excel) 3. Excellent oral and written communication skills. 4. Good organizational, analytical, and problem-solving skills. 5. Ability to function effectively with a wide variety of disciplines, including physicians and nurses. 6. A team player that can follow a system and protocol to achieve a common goal. 7. Confidence to communicate and collaborate with other community health care organizations and personnel. 8. Able to participate as part of a team to accomplish an assigned project. Preferred: 1. Previous experience in a Primary Care Setting. 2. Previous experience in case management, care coordination, or patient navigation
